Emphasis added:

Effective this Friday, August 20th, all guests dining indoors at Perbacco will be required to present proof of full vaccination upon arrival.

Please bring one of the following items, along with a valid government-issued photo ID:

  1. Original copy of your COVID-19 Vaccination Record Card (issued by the US Centers for Disease Control & Prevention) or WHO Yellow Card, which includes name of person vaccinated, type of vaccine provided, and date last dose administered); OR

  2. a photo of your Vaccination Record Card as a separate document; OR

  3. a digital photo of your Vaccination Record Card stored on an electronic device.
